Partnering Tool for Transitions in Care (TiC) – Team Grants Phase 2

The main objective of this tool is to enable partnerships between researchers, individuals and organizations interested in sharing available resources and identifying areas of interest to forge collaborations in relation to the Transitions in Care – Team Grants Phase 2 Funding Opportunity. The information provided will be shared publically on CIHR’s web page, which will be updated weekly.
