Employee Assistance Services - Client Survey
YOUR OPINION IS IMPORTANT!
In an effort to improve your Employee Assistance Program (EAP*), we need your help in evaluating the service offered to you. Your feedback, while voluntary, is important and will help us improve the quality and effectiveness of your EAP. Please give us your honest opinion, whether it is positive or negative, and respond to all questions. The results will be used to improve our services, to assess our clients’ satisfaction level and to report the impact of the program to our senior management and client organizations.
Your answers will be strictly anonymous and confidential. The personal information you provide to the EAP of Health Canada is handled according to the Privacy Act and is being collected under the authority of section 4(2)(f) of the Department of Health Act.
* Please note that EAP is a generic acronym for the program offering services to: Public Service employees covered by EAS, members of the Canadian Forces (CF), former military members or veterans, CF and Royal Canadian Mounted Police (RCMP) retirees covered by Veteran Affairs Canada (VAC), as well as their immediate family members.
